You must also submit a completed Medical Exemption Form and/or Disability Exception Form, a Deferrral During Preganancy Request Form or a Religious Exception Request Form to the Student Health and Counseling Service’s (SHCS) COVID-19 nurse via secure message to avoid a registration hold for non-compliance.All requests for exceptions will be reviewed and either approved or declined by the appropriate UCSF department in approximately three business days. To request an exception, go to the OHS website to download, complete and upload a form.
